Persist SecurityService PHI access trail to phi_access_log
The §164.312(b) trail was going to Slf4jPhiAccessLogWriter: 14-day platform retention against a six-year obligation, and nothing that can be queried. The lead reads and CSV/HMDA exports this service records are the accesses a §164.528 accounting is assembled from, so the trail has to survive in a table. Swaps in JpaPhiAccessLogWriter via a bean PhiAccessRecorder picks up through ObjectProvider.getIfUnique — no call site in PhiAccessAuditor changes. The matching com.kamo.z.shared.phi.audit entry in @EnableJpaRepositories ships in this same commit: the bean cannot be constructed without the repository, and declaring one without the other is what CrashLooped login earlier today.

Complete a login that stopped at the second factor
SecurityService now gates login on a second factor (§164.312(d)). When one is owed it creates the session but withholds its one-time key, returning a short-lived challenge token instead — a session nobody holds an OTK for is unreachable. This BFF expected an oneTimeKey unconditionally, so an MFA challenge fell into the "no session key received" branch and surfaced to the user as a 500. It now recognises the challenge and passes mfaRequired/mfaToken to the client. /api/login/mfa completes it. From the OTK onward it reuses the existing login route's handoff verbatim — resolve *** from Redis, clear stale cookies, set the new one — because two session-handoff implementations would drift and the one used less often would be the one that rots. A wrong code returns 401 with mfaRequired still true and the same mfaToken: SecurityService deliberately does not consume the challenge on failure, so the user retries the code rather than the password. Forcing a re-password over one typo is how people end up turning MFA off. Inert until someone enrols; nobody has.
· VOIPService
Persist the VOIP PHI trail to phi_access_log
Call recordings, voicemail audio and transcripts were audited into the log stream, which keeps 14 days against a six-year §164.528 obligation and cannot be queried by resource — so "who listened to this call" stayed unanswerable even while it was being recorded. Wiring JpaPhiAccessLogWriter as the writer bean displaces the Slf4j fallback that PhiAccessRecorder resolves through ObjectProvider.getIfUnique, so CallRecordingAuditor's existing call sites are unchanged. The @EnableJpaRepositories entry ships in this same commit by necessity: the bean needs PhiAccessLogRepository, and the explicit scan list is the only thing that creates it. Declaring one without the other fails context startup.
· AIService
Persist AI conversation PHI access trail to phi_access_log
Stored assistant conversations are the reads least able to afford a 14-day, unqueryable log stream: the module sits outside the compliance boundary, so prompts about borrowers are persisted indefinitely and a §164.528 accounting has to be assembled from exactly this trail six years later. recordDisclosure already fails closed on a write it cannot make, which only means something once the write lands somewhere durable. Declares the JpaPhiAccessLogWriter bean and adds com.kamo.z.shared.phi.audit to @EnableJpaRepositories in the same commit. The library leaves the writer unannotated so ~40 services do not all construct it, which makes the bean and the scan entry a single indivisible change — the bean alone leaves PhiAccessLogRepository with no bean definition and CrashLoops the service.

Endelea Huduma ya Vyombo vya Habari PHI upatikanaji wa njia ya phi access log
Chat message reads were recorded through the Slf4j default writer, which puts the §164.312(b) trail in a 14-day log stream against a six-year retention obligation — and leaves "who read this conversation" unanswerable, since a log stream cannot be queried by member or by resource. The JpaPhiAccessLogWriter bean and the com.kamo.z.shared.phi.audit entry in @EnableJpaRepositories ship together deliberately: @ComponentScan("com.kamo") picks up the @Configuration, but the repository list is explicit, so the bean without the scan entry means PhiAccessLogRepository has no bean and the context fails to start. ChatMessageAccessAuditor needs no change — PhiAccessRecorder resolves its writer via ObjectProvider.getIfUnique, so the JPA writer displaces the Slf4j fallback at every existing call site.

· DocsService
Persist this service's PHI audit trail to the database
DocsService serves document bytes, so its trail is what an accounting of disclosures under §164.528 is actually assembled from. It was writing to a 14-day log stream against a six-year obligation, with no query surface. The bean and the @EnableJpaRepositories entry land in the same commit deliberately. JpaPhiAccessLogWriter is unannotated in the library precisely so services opt in one at a time; declaring the bean without the matching scan is what took every login on the platform down earlier today, and separating these two edits across commits would reproduce that failure exactly.

Durable audit trail — persist PHI access, append-only
Turns §164.312(b) from a log line into evidence. The capture layer has been wired across seven services for a while, but it wrote through Slf4jPhiAccessLogWriter — 14-day retention against a six-year obligation, and unqueryable. The control was architecturally present and evidentially useless. PhiAccessLog is append-only: @PreUpdate and @PreRemove refuse mutation, because an audit trail the application can edit is not evidence. The database half (REVOKE UPDATE/DELETE) is in the KamoInitializer migration. It carries identifiers and never content, and a test asserts the entity has no content-shaped field. A trail that reproduces what it audits has simply relocated the PHI into a second store — one that by design is never deleted. JpaPhiAccessLogWriter is deliberately NOT annotated as a bean. Every service component-scans com.kamo, so a stereotype would make all ~40 construct it, and since it needs a repository each would then have to add this package to its own explicit @EnableJpaRepositories list or fail to start. That exact mistake took every login on the platform down earlier today. A service that wants durable audit declares the bean and adds the scan; the rest keep the Slf4j default unaffected. SharedLibBeanSafetyTest enforces this. Fields are truncated rather than allowed to throw. On a fail-closed access kind an exception here refuses the user's read, so a pathological URL or 4KB user-agent losing the row would also deny legitimate access — the audit layer must never be the thing that breaks the product. Table already exists in the cluster (KamoInitializer fb89b3d), so nothing is armed.
· InitializerService
Create phi_access_log — the durable §164.312(b) audit trail
Until now PHI access was recorded only to a log stream with 14-day retention against a six-year obligation, and with no query surface. An accounting of disclosures under §164.528 is a query; stdout cannot be queried. Indexes are chosen for the three questions actually asked: everything that touched one record, everything one workforce member touched in a window, and everything in one tenant in a window (the §164.308(a)(1)(ii)(D) activity review). REVOKE UPDATE/DELETE is the database half of append-only; @PreUpdate/@PreRemove on the entity is the application half. Either alone is weak — the entity guard is bypassed by raw SQL, a grant can be re-added — but together the running application cannot quietly rewrite an audit row. Best-effort: CockroachDB lets a table owner retain privileges, so a failure is logged rather than fatal. Table verified present with all 14 columns and all three indexes before the entity was pushed, so no service can build against a mapping with no table.

Mint the Universal Loan Identifier (ULI) + store the org LEI
Adds the HMDA ULI to the LAR export. UliMinter builds the ULI = org LEI(20) + loan number + two ISO 7064 MOD 97-10 check digits (Mod97CheckDigit — the IBAN algorithm, transcribed from Reg C 12 CFR 1003 appendix C and pinned against its worked example, NOT the MERS Luhn variant). New nullable Organization.lei column (KI-applied) with an org-patch write path; HmdaLarService emits the ULI when the org has a valid LEI + a loan number, else DEGRADES to the bare loan number so a malformed ULI the CFPB platform would reject is never written. LoanIdentifierType gains UNIVERSAL_LOAN_IDENTIFIER (derived: org LEI + LoanFile.loanNumber + check).

Set the org LEI in Company Info (activates the HMDA ULI)
The Company Info card gains a Legal Entity Identifier field (20-char, upper- cased), sent through the existing org PUT — OrganizationController deserializes it onto Organization.lei and OrganizationService applies it. With an LEI set, HmdaLarService mints the borrower's Universal Loan Identifier; without one it degrades to the bare loan number, so this is what turns the ULI on per org.

Gate login on the second factor — §164.312(d)
Completes MFA. The core landed in kamo-shared-library; this is the wire that turns stored configuration into an actual gate. The gate is small because login already had the right shape: it creates the *** session and hands it to the client ONLY as a one-time key in the response body — no cookie is set at login. So the entire mechanism is to withhold the OTK until the second factor is proven. A session nobody holds an OTK for is unreachable, which means session creation itself needed no surgery at all. MfaChallengeService holds the ***Id under an opaque, single-use, 5-minute token. The client never receives the session id, and the token is worthless without a code. Short on purpose: a half-authenticated state survives a password compromise. POST /api/security/mfa/challenge completes the login, releasing the OTK so the client resumes exactly the flow it would have had without MFA. A wrong code releases nothing and — deliberately — does NOT burn the challenge, because forcing a user back through the password over one typo is how MFA gets switched off. The enrollment's own lockout counts those failures. A success consumes the challenge so it cannot be replayed. Recovery codes complete it too, and are never also tried as a TOTP. Enrollment endpoints (/enroll, /confirm, /status) require a session — enrolling a factor is an authenticated action on your own account. /challenge deliberately does not, because its caller is mid-login by definition. INERT TODAY: the gate fires only on hasConfirmedFactor, and nobody is enrolled, so the existing login path is byte-identical. That is the point — this can deploy before any client supports it. The endpoint ratchet caught my own three enrollment handlers as unguarded. They are guarded, through a userIdFromSession helper the one-method-deep scan cannot follow, so the fix was teaching GUARD that helper rather than baselining them. I also documented the converse limitation the same exercise exposed: completeChallenge clears the scan only because it happens to touch kSessionService.

TOTP second factor — §164.312(d)
The compliance assessment called the absence of any second factor disqualifying on its own, and under the 2025 Security Rule NPRM MFA stops being addressable at all. Single-factor username+password was the only interactive authentication path on the platform. TOTP is verified against the RFC 6238 Appendix B test vectors rather than against values this implementation produced. That distinction is the whole ballgame: a home-grown TOTP that agrees only with itself authenticates nobody's Google Authenticator, and the failure surfaces at rollout with every user locked out. Writing those tests immediately caught that I had passed unix seconds where the HMAC wants a counter — off by a factor of thirty — so generateAt now does the division and no caller has to remember it. Properties that separate real MFA from the appearance of it, each pinned by a test: - Codes are single-use. A TOTP stays valid for its entire 30-second step, so without lastUsedTimeStep an intercepted or shoulder-surfed code works again inside its own window. An earlier step still inside the tolerance is refused too, once a later one has been accepted. - Unconfirmed enrollments do not count. Beginning enrollment and abandoning it before proving possession must not satisfy an MFA requirement. - Lockout is real: a correct code is refused while locked, otherwise the lockout is decorative. - beginEnrollment refuses to replace a confirmed factor. Silently overwriting it would let anyone reaching the endpoint swap the victim's authenticator for their own. - The secret is encrypted at rest and returned exactly once; recovery codes are hashed and single-use. Neither is recoverable by staff, so a lost device means re-enrollment — the correct trade for a credential that bypasses the password. - Verification compares in constant time. String.equals returns on the first differing digit, which leaks how many leading digits were right and collapses a 10^6 space into roughly 60 guesses. Recovery codes are SHA-256 rather than bcrypt deliberately: 80 bits of machine-generated entropy has no dictionary to stretch against, and a slow hash there would only buy a denial-of-service on the login path. Tables already exist in the cluster (KamoInitializer c65a233), so nothing is armed by this push. Enrollment and challenge endpoints are the next step; nothing calls this yet.

Create the second-factor tables
Backs UserMfaEnrollment and MfaRecoveryCode in kamo-shared-library — §164.312(d) Person or Entity Authentication, the gap the compliance assessment called disqualifying on its own. Both tables verified present in the cluster with the expected shape before the entities were pushed, so no service can build against a mapping whose table does not exist. Three details worth not losing to a later simplification: - SECRET_CIPHERTEXT holds ciphertext. The service refuses to write an enrollment at all when no key is configured, rather than degrading to plaintext. - LAST_USED_TIME_STEP is what makes a code single-use. A TOTP is valid for its whole 30-second step, so without it an intercepted code replays inside its own window. - Recovery codes get a row each rather than a serialized list, so redeeming one is a targeted update instead of a read-modify-write over the set — which under concurrency is exactly how a "single-use" code gets used twice.

Require VIEW_CALL_LOGS for call playback in PHI-handling orgs
Recording and voicemail playback checked tenancy and nothing else: any member of the owning org could pull any call in it, transcript included, on extensions that were not theirs. Requiring VIEW_CALL_LOGS outright was not an option — a right can exist in the enum and be granted to nobody, so switching it on for everyone takes the softphone away from the members using it today. Enforcement is therefore scoped to the compliance boundary that makes it necessary. For an org with handlesPhi set, recording audio, per-leg tracks, voicemail audio and the transcript riding inside recording metadata require the right; the refusal is a 403 and lands in the PHI audit trail through the existing denied-path recorders, so probing is visible. For every other org — all of them today — nothing changes, which is what the **************** cases pin down. Known consequence: the ?st= stream-token session carries only (orgId, memberId), so under a PHI tenant the audio-tag path fails closed until the token carries the grant. Recorded in a test rather than left to be discovered.

Automatic logoff after inactivity (HIPAA 164.312(a)(2)(iii))
A session left open on an unattended workstation stayed usable indefinitely. This terminates it after 15 minutes of no interaction, with a 60-second warning so nobody is dumped mid-task. The timing rules live in app/lib/idleTimeout.ts as a pure controller (unit tested with fake timers); the component is only an input layer. Three decisions worth knowing about: - Only pointer, key, wheel, scroll, touch and visibility events count as presence. Websocket frames, polling and timers do not — kamo-internal screens refresh themselves constantly, and treating that as a human at the desk would make the control decorative. - Presence is measured across the whole origin, because the control's subject is an unattended machine, not an unattended tab. Work in one tab keeps its siblings alive; when every tab goes quiet they expire together, so no arrangement of tabs leaves a usable session on an abandoned screen. - The end of a session is announced across tabs. The *** id lives in each tab's sessionStorage over one shared Redis session, so a sibling that is never told keeps showing an authenticated workspace over a session that no longer exists. Both the automatic logoff and the ordinary /logout page now broadcast. Termination goes through the existing /logout page — SecurityService logout, Redis key deletion, cookie clearing, forward to kamo-login — because clearing client state alone leaves the server session alive and is not a logoff. Copy shipped ahead of this in kamo-translation-dictionary (c39dfb9).

Audit WOPI content endpoints and close the last byte-serving gaps
WOPI serves whole documents to Docs and recorded nothing. GET /wopi/files/{id}/contents is the same disclosure as /download, relayed the same way through APIService, with no trail at all — opening a document in the editor was untraceable. Identity comes from the WOPI access token, not a *** session: DocumentController mints it with the opening member's org and member ids and Docs returns it on every callback, so those claims are the only identity these endpoints have, and the one checkFileInfo already authorizes against. The auditor takes them directly rather than resolving a member a second way that could disagree with the token the request was authorized by. A token that cannot be read records against the unresolved-org sentinel instead of being dropped. PutFile records as VIEW, not DOWNLOAD: a save sends bytes in rather than out, so calling it a disclosure would double-count the GetFile that opened the session, and VIEW fails open so a sick audit sink cannot destroy unsaved work. CheckFileInfo and the lock operations stay unrecorded — no content, and Docs calls both repeatedly through one session, which would bury the row that means bytes actually moved. The sweep for remaining byte-serving paths found two more: - /api/docs/binders/{id}/render merges every document in a binder into one PDF. It is bulk-download in a different wrapper and recorded nothing; now one event per embedded document, separators and generated pages excluded. - **************** wrote only an ImgLogDownload row. That is an imaging-domain record read by the download-history panel, not the cross-surface trail — so the accounting of disclosures showed streams, exports and template PDFs while omitting the service's most-used byte path. Both records are now written; they answer different questions. No entity, @Column or shared-library change.

The pipeline already carries five separate hand-rolled line-item editors (orders/new, OrderEditDialog, the subscription order grid, the procurement table, work-order labor/parts) with nothing shared between them; new work should build on this rather than adding a sixth. The editor's totals are a LOCAL PREVIEW and are labelled as such while dirty. They mirror QuoteTotals exactly — line discounts first, then the quote-level discount, then tax and shipping — so the preview never disagrees with the saved document, and the server's response replaces them on every save. Nothing here can set a total. Tax stays honest in the UI too: ENGINE mode renders "Not configured" rather than a fabricated figure, matching what the API reports. A freshly minted share link is shown once with a copy-now warning, because only its SHA-256 is persisted and the URL genuinely cannot be recovered. The list can show a token prefix and nothing more. Rights-gated throughout: MANAGE_QUOTES to create/edit, SEND_QUOTES to mint or revoke links. Terminal quotes **************** lock the editor and offer a revision instead.
